Veterans Home Care (VHC)

Care Direct is pleased to announce that we have been awarded a tender by the Department of Veterans’ Affairs, to provide Veteran Home Care (VHC) services to veterans across Queensland.

Our program is dedicated to providing essential support and assistance to veterans through our Veterans’ Home Care (VHC) services. Our goal is to enable veterans to continue living independently by offering practical help tailored to their unique needs. Here's an overview of the services we provide, who is eligible, and how to access them.

WHO CAN RECEIVE OUR SERVICES:

Veterans with a GoldCard or a White Card for an accepted service-related injury or condition.

Carers and family members of Gold Card and White Card holders with an accepted service-related condition may also be eligible for assessment.

Services We Offer

  • 1. Domestic Help: Our domestic help services encompass house cleaning, unaccompanied shopping, and bill paying.

    2. Personal Care: We assist with essential personal care tasks such as showering, toileting, and eating.

    3. Respite Care: Our respite care options include in-home respite, residential respite in a government-subsidized aged care facility, and emergency short-term home relief (ESTHR).

    4. Home and Garden Maintenance: For home and lawn maintenance, we cover tasks like replacing light bulbs, cleaning gutters, ceiling fans and windows, and even non-safety-related lawn mowing.

    5. Social Assistance: CVC Social Assistance is a 12 week service designed to (re)connect socially isolated CVC participants into community life. The service is designed to empower the CVC participant to meet their need for social contact and/or company.

    Service Allocation

    The allocation of services is determined based on individually assessed clinical needs. This could range from up to 196 hours of in-home respite care, 28 days of residential respite care, to 15 hours of home and garden maintenance annually.

    ACCESSING OUR SERVICES

    Accessing our services is a straightforward process:

    1. Call the VHC Assessment Agency at 1300 550 450 for a phone assessment.
    2. The assessment results in a personalised care plan.
    3. A VHC service provider will contact you to schedule suitable service times.
    4. Reassessments take place every 6 to 9 months to ensure continued tailored support.

    Veteran Healthcare Cards and Eligibility

    DVA (Department of Veterans’ Affairs) can issue cards that give you access to certain benefits. The benefits depend on the card type. You may get free or cheaper health care and support, discounts on medicine, transport, and water and power bills. Some businesses may also give discounts.

DVA Community Nursing

DVA Community Nursing

The Department of Veterans’ Affairs (DVA) provides a variety of in-home and community support programs to assist eligible veterans and war widows/widowers (Entitled Persons) to continue living in their home.

Community Nursing services are provided to entitled persons to meet their clinical and personal care needs.

Entitled persons include veterans and war widows who hold a Gold Card, and also those veterans with a White Card may be eligible for the service where the condition requiring nursing is accepted by DVA as war-related. The aim of the program is to enhance the independence, quality of life and health outcomes of veterans and their widows by avoiding early admission to hospital and/or residential care through access to high-quality community nursing services.

How Can I Receive DVA Community Nursing?

We'll help you every step of the way

To receive DVA community nursing, the entitled person must obtain a referral from their GP or the Hospital Discharge Planner. Following this referral, Our DVA Care Coordinator will liaise with you to arrange a convenient time to complete a comprehensive community nursing assessment by a highly experienced and dedicated Registered Nurse.

Care Direct offers a wide variety of Community Nursing services designed to maintain the health and well being of the entitled veteran or widow. The services offered will depend on the client’s assessed individual care needs.

We believe it is important for service and support to be designed and developed around the needs, values and aspirations of the people receiving services and their families.

Accessing DVA Community Nursing Services Is Available Through Referral From Your GP Or The Hospital Discharge Co-Ordinator.

How Can I Access This Service?

We will contact you as soon as we receive your referral, and we arrange to come to your house to complete a full nursing assessment and discuss care plan.

Eligibility

To be eligible to receive community nursing services, you must be a Gold Card or a White Card holder, and undergo an assessment to determine your need for clinical and/or personal care.

Costs

There is no cost to you if you are assessed as eligible for a DVA Community Nursing Program.
